Applicants seeking a burial flag should:
Submit a completed VA Form 27-2008 and attach a copy of the veteran's DD-214, Certificate of Release or Discharge from Active Duty to your local Post Office™

Request for Funeral Honors
The funeral director will call in the request and forward the documentation (FS Form 733) by facsimile to the Casualty Assistance Office. All funeral requests should be received 48 hours in advance of the funeral service.

Mortuary Affairs
The Mortuary Affairs Coordinator ensures the proper care and disposition of a Soldier's remains and personal effects. Additionally, prepares orders for eligible survivors to travel at government expense to their Soldier's funeral and/or unit memorial.

Memorial Affairs
Coordinates military funeral honors for eligible deceased veterans, retirees and active duty Soldiers in the Fort Sill area of responsibility. FS Form 733 and DD Form 214 is faxed to 580-442-6914.

Trainers
Casualty Notification/Assistance Officers must attend a three-day certification course prior to performing these services. The CAC's training instructor offers this course twice every month.

Benefits and Entitlements
Surviving spouses of active and retired duty Soldiers may receive casualty assistance. Benefits Coordinator processes benefit claims, coordinates services from support organizations, and answers questions related to entitlements.

Dependent Deaths
All military dependents are reportable deaths; when a dependent becomes deceased, report the death to the CAC immediately.

Death of Retired Soldier
The death of a Retired Soldier must be reported through official channels. Normally, this information is obtained from the funeral director when Honors are requested.
